How to fill this in
Enter the receipt number, the date received, the payer, the recipient, and the amount you were paid. The currency starts as USD. Add the payment method and a short purpose when you have them. A blank amount is not printed as zero.
If sales tax was part of the payment, type that rate. The PDF shows it as included in the amount received and does not add it again. State and local rates are not looked up. Leave the rate empty when the receipt has no tax line.
Choose A4 or US Letter before you download. Empty dates, rates, and business numbers stay empty.

Is sales tax added on top of the amount received?
No. The amount you type is the amount received. A rate only identifies tax already inside that figure.
Is this a credit-card slip?
No. It is a payment receipt made from what you type. It does not talk to a card network.
Does one sales-tax rate apply?
