Write the employer, the candidate, the job title, the place of work, the start date, and the pay, including how often it is paid. The currency starts as USD. Put the state in the address. A reply date, hours, and leave stay blank until you enter them.
Employment rules differ by state. Montana's Department of Labor says Montana is not an at-will state, and a discharge after probation can be wrongful without good cause.
